*Extended Soft Tissue Treatment / Therapeutic Exercise can only be added to a regular visit and cannot be purchased as a stand-alone visit.

Additional visits beyond what are included in the monthly membership are charged at a rate of $99/visit for Maintenance Membership and $75/visit for Active Care Membership.

Memberships can be cancelled at any time, as long as it is prior to the next month's charge processing.

Unused visits roll over as long as membership is active and continuous.  If membership is cancelled, all unused visits are forfeited.
